General
What is direct primary care?
It means you pay a membership directly to Sorrel instead of billing insurance for each visit. That's what buys the time: hour-long visits, same or next day access, and a doctor who calls and texts you back instead of routing you through a portal. More on how direct primary care works.
Is Sorrel a primary care practice?
Yes. Sorrel is a direct primary care practice for women 18 to 64. We provide primary care, including preventive care, annual exams, acute and chronic condition management, medication management, lab interpretation, coordination of care and referrals, as well as perimenopause, menopause, and metabolic health support.
Do I have to become a member to see you?
No. Sorrel offers both membership care and fee-for-service care. Membership is designed for patients who want the most access, continuity, messaging support, and member discounts. Fee-for-service care is available for patients who prefer to pay per visit.
Fee-for-service initial consultations are $500, and follow-up visits are $300. Messaging access and discounts on labs and Fullscript are more limited than they are for members. A superbill can be provided upon request for non-member visits.
Who is welcome at Sorrel?
Sorrel is designed around the health needs of women, and welcomes transgender men, nonbinary people, and other gender-diverse patients whose care needs fit the practice's scope.
That includes people medicine has a track record of failing: patients in larger bodies, LGBTQ+ patients, neurodivergent patients, and people living with disability or chronic illness.
Coverage, billing, and insurance
Do you take insurance?
No. Sorrel is a direct care practice and does not bill insurance. Members and fee-for-service patients pay Sorrel directly for care. HSA and FSA cards are accepted and itemized receipts are available on request.
Is this health insurance?
No. A Sorrel membership covers your care with Dr. Tierney. It is not insurance and it does not replace it. Most members keep a plan for hospital care, surgery, and specialists.
Can I still use my insurance for labs and imaging?
Yes. Labs, imaging, and prescriptions are billed the usual way, so your insurance, HSA, or FSA still applies to them. The membership covers the time with your doctor, not the outside services she orders.
Do you see Medicare patients?
Sorrel does not currently provide care for Medicare patients, but Medicare patients may join the waitlist.
Can I cancel my membership?
Yes. Memberships bill monthly and cancel with 30 days notice. Pausing is fine too.
For visits, we ask for 48 hours notice to reschedule. Inside that window there is a $50 charge, and non-members forfeit the visit fee. Life happens and we try to be flexible, but late cancellations in a small practice mean someone else couldn't be seen.
What we treat
What do you actually treat?
Primary care. Prevention, blood pressure, diabetes, thyroid, acute illness and minor injury, mental health support, reproductive health, and ongoing care coordination.
Cycle and hormonal life stages. Menstrual and cycle-related concerns, PMDD, and contraception, alongside the changes in mood, sleep, energy and metabolism that come with each stage.
Menopause and hormone health. Perimenopause and menopause symptoms including hot flashes, brain fog, mood changes and sleep disruption. Hormone therapy with estradiol, progesterone and testosterone. Sexual health and genitourinary syndrome of menopause.
Metabolic health. PMOS (formerly known as PCOS), insulin resistance, metabolic syndrome, weight-inclusive obesity care, GLP-1 management, and full metabolic assessment.
Complex and hard to pin down. MCAS, POTS and hypermobile Ehlers-Danlos, the conditions that get missed when care is split across four specialists who never talk.
Do you prescribe hormone therapy?
Yes, when appropriate. We offer evidence-based menopause and perimenopause care, including hormone therapy when it fits your symptoms, goals, health history, and risk profile. Sorrel does not prescribe or place hormone pellets.
Do you prescribe weight-loss medications?
Yes, when appropriate. Weight-loss medication, including GLP-1 medications, can be part of care for patients who want that support and for whom it's medically appropriate.
Do you prescribe compounded GLP-1s?
No. Sorrel does not prescribe compounded GLP-1 medications. If you're already getting them elsewhere, Dr. Tierney can provide supportive guidance so you use them safely.
Is Sorrel weight-neutral?
Yes. Sorrel is weight-neutral across the practice, including primary care, menopause care, metabolic health, and weight management. That means care is grounded in respect, body autonomy, and informed choice. We do not use shame, coercion, or one-size-fits-all weight loss goals.
If you don't want to lose weight, that's fine too. Metabolic health is about far more than a number on a scale. Care here centers on what matters to you: energy, strength, longevity, and feeling at ease in your body.
Do you see patients with POTS, MCAS, hypermobility, hEDS, long COVID, or chronic fatigue?
Yes, patients with complex or hard-to-explain symptoms are welcome here. We take time to understand your history, review what has already been tried, evaluate for common contributors, coordinate referrals when needed, and build a practical plan for primary care, symptom support, and next steps.
Access and logistics
What happens if I need urgent care or a specialist?
Call or text Dr. Tierney first. A lot of what sends people to urgent care can be handled same day by a doctor who already knows your history. When you do need a specialist, she makes the referral and stays involved rather than handing you off.
Can I see you virtually?
Yes. Sorrel offers virtual care across Washington state and in-person care in Seattle.
