You may already have a primary care doctor you like. What you may not have is someone who knows menopause and metabolic health well, and has more than fifteen minutes to spend with you. You can work with Dr. Tierney on either of those without becoming a member and without leaving the doctor you have.
Menopause and perimenopause care
You're waking up at three in the morning. Your body and mind feel unpredictable.
As a Menopause Society Certified Practitioner, Dr. Tierney will discuss your experience, the evidence around treatment, and make a plan together.
What a menopause visit covers
- A full history, not a symptom checklist. Your sleep, cycle, mood, energy, sex life, brain, and body, and what's actually driving what.
- Labs when they'll change the plan, and a straight answer when they won't. Perimenopause is usually diagnosed by story, not bloodwork.
- Menopausal hormone therapy. Estrogen and progesterone when they're right for you, with benefits and risks discussed for your history. Testosterone when appropriate.
- Non-hormonal options for hot flashes, sleep, and mood, if hormones aren't a fit or aren't what you want.
- Vaginal, urinary, and sexual symptoms, including local estrogen, which is safe for nearly everyone and wildly under-prescribed.
- The long game. Bone density, heart health, and cognition, and what the evidence actually supports.
Metabolic health and weight management
For women who want dedicated support with metabolic health, including intentional weight loss. Dr. Tierney is board certified in obesity medicine, and practices it without diet culture. The goal is a body that feels strong and well.
What this care includes
- A real look at your metabolic picture. Blood sugar, insulin, cholesterol, blood pressure, thyroid, and PMOS (formerly known as PCOS) when it's part of the story.
- Medication management when it's the right tool, including GLP-1 medications, with honest talk about what they do, cost, side effects, and the long term.
- A nutrition plan that works in real life. Protein, fiber, and enough to eat. No food is off limits and nothing here depends on willpower.
- Strength, sleep, and stress, the parts of metabolic health that don't come in a prescription.
- Visits about every four weeks at first, that's what it takes to adjust a plan safely. Once you're steady, we space them out.
Your weight may change. It isn't how we measure whether this is working. How you feel, how you're sleeping, how strong you are, and what your labs show tell us more.

How visits and pricing work
You pay per visit. No membership, no monthly fee, no contract.

If you'll be here regularly, Primary Care+ membership includes menopause care and metabolic health and weight management for $175 to $250 a month, less than a single follow-up.

Do I have to leave my current doctor?
No. Sorrel is designed to work alongside your current doctor, not replace them. Dr. Tierney sends visit notes to your primary care doctor so everyone's working from the same information.
Do you take insurance?
No. Sorrel is cash-pay, which is what buys you a full hour and direct access. Keep your insurance for labs, imaging, prescriptions, and everything else.
Can I switch to membership later?
Yes, any time. Membership covers your primary care, menopause care, metabolic health and weight management, and ADHD, all under one monthly rate. If you switch within 30 days of your first visit, we'll waive the $150 registration fee and your first month's membership.
What if cost is the barrier?
We hold a small number of need-based membership spots at a reduced or waived monthly fee. If cost is the only thing standing between you and the care you need, reach out about joining the waitlist.
